The Home Health RN Case Manager position is a critical position for ensuring patients get the care they need in their homes. Our offices are in the Arboretum at Midlothian Turnpike and Powhite Parkway; we serve the Richmond- Petersburg metro area and have a growing referral network. 

As a Case Manager RN,  you will organize and direct home care services for our patients; we look for nurses who are experienced in delivering quality nursing care; while home health experience is preferred, we welcome applications from RNs with critical care and medical/surgical experience in other settings. You will also supervise other clinical personnel and serve as the prime point of contact for the patient while under the care of James River Home Health and Hospice.

Responsibilities: 

Patient Care

Acts as Case Manager and assumes responsibility for coordinating patient care for patient assigned as part of the Case Manager's caseload. This includes the following responsibilities:

  • Completes an initial assessment of each patient and his/her family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Regularly re-evaluates patient nursing needs.
  • Initiates the plan of care and makes necessary revisions.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate.
  • Identifies discharge planning needs as part of the care plan development.

Communication

  • Prepares clinical notes and updates the primary physician.
  • Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
  • Communicates with community health resources to coordinate the care plan.

Additional Duties

  • Participates in on-call duties as appropriate.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Instructs, supervises and evaluates home health aide care.
